Patient Feedback

Patients and others have opportunities to register their
complaints either verbally to staff, in email to management Nikki  phmcpracticemanger@gmail.com or via our suggestion box.

Patients should feel free to complain anonymously if desired. Complaints as they arise will be forwarded to management.

If you feel the matter has not been resolved you can contact the Health Complaints commissioner.

Patient Communication Policy

Patients are encouraged to reach out to their healthcare providers for questions, concerns, or appointment scheduling. However, to maintain confidentiality and protect personal health information.

Phone Communication:

Patients can call the clinic/office during business hours for any inquiries or to schedule appointments.

For urgent medical concerns, please call the office directly or seek emergency care.

In-Person Visits
For discussions that require more privacy or detailed medical information, please schedule an in-person appointment with your doctor.

For your safety, please do not send personal health information, including but not limited to medical history, symptoms, or test results, via email.

Email is not a secure method of communication and may not protect your privacy.
